From personal experience to a shared tool.
PACientActiu was born from a first-hand need: to keep track of what happens during a long recovery, understand its progress better and be able to explain it more clearly.
What began as a personal response has become a shared proposal: a way of taking an active part in health, supported by tools built from real needs.
Remember what happens between visits.
Symptoms, habits and small changes can feel far away by the time of the appointment. Recording what matters helps you overcome forgetfulness and avoids having to rebuild weeks or months from memory alone.
Bring a clearer story to your appointment.
An organised record of your progress makes it easier to explain what has changed, how often and in what context. It does not replace the clinical interview: it offers a more complete starting point for the conversation with the professional.
Take part without replacing the professional.
Being an active patient does not mean diagnosing yourself or making clinical decisions alone. It means observing, asking better-informed questions and taking part in decisions with the support of those who care for us.
Build knowledge, not just data.
Collecting records is not the goal. The value appears when data is shown with progress and context, possible correlations are kept apart from evidence, and clinical interpretation is left to those whose role it is.
Data under the person’s control.
Privacy, consent and the ability to decide what is shared have been part of the project from the start. Each tool must explain transparently where the data is kept and who can access it.
Built with patients and professionals.
PACientActiu needs to listen to diverse experiences, validate which information adds value and adapt its tools to real care processes. Collaboration with patients, associations and professionals is central to the initiative.
A personal need can become a useful tool for many people.
The project is still in development and clearly distinguishes what already exists, what is a prototype and what is part of the future vision.
